Enrolling your student at DCS takes just 3 steps!

Start Your Application
Submit this quick form to get the application process started and hold a seat for your student.
STEP 2 OF 3
Gather Documents
Now that you’ve started your application, it’s time to gather all of the required documents. Please be sure you have all of the following:
1. Birth certificate
A birth certificate or record of birth for each child you’re enrolling
2. Latest grades
• K-8: A current report card for each child you’re enrolling
• High School: A current transcript for each child you’re enrolling
3. Shot records
Shot records for each child you’re enrolling
3b. Physical (Kindergarten only)
For each Kindergarten student you’re enrolling, a current health appraisal form (physical) is required
4. Parent / Guardian’s ID
Your valid state ID or driver’s license

- Hurry in! Your student’s seat is only held for 10 days after you start the application (from Step 1).
- Be sure to bring all of the required documents (from Step 2).
